20. Relationship with next of kin:
21. Append your signature on a white paper then send it.
22. Business Category(Other business you do)..
**Kindly note that if your business is registered with CAC and you intend to retain your Business Name, you will have to also send us your CAC Certificate, the CAC Application form, and other relevant documents.
If the above is not provided, it means the account will be opened with your BVN name pending when you submit the CAC documents.

Also, note that our terminals are not for sale. The amount paid is for Activation and Onboarding fees and if you return the terminal or it's being retrieved (due to INACTIVE), only #10,000 is refundable provided it's within one year of activation...
