As a Support Worker you'll help people remain independent in the comfort of their own homes by providing:
• Personal care and support.
• Companionship and emotional support.
• Assistance with medication.

• Light household duties.

• Encouraging independence while promoting dignity and respect.

• Late Shift: 3:00pm – 10:00pm
• Night Shifts

This is an excellent opportunity for an experienced care professional who is ready to take the next step in their career, helping to ensure outstanding care is delivered while supporting and developing our team of carers.

Your Responsibilities
As a Team Leader, you will play a key role in the day-to-day management of our care service, including:
• Preparing and managing staff rotas.
• Supporting and supervising care staff.
• Completing and reviewing care plans.
• Carrying out client risk assessments.
• Participating in the on-call rota to support staff and clients outside normal office hours.
• Ensuring care is delivered to the highest standards.
• Maintaining accurate records and documentation.
• Liaising with clients, families and healthcare professionals.
• Promoting safe working practices and ensuring compliance with CQC standards.

What We're Looking For
Essential:
• Level 3 Diploma in Health & Social Care (or equivalent).
• Previous experience working within domiciliary or community care.
• Excellent communication and organisational skills.
• A caring, compassionate and professional approach.
• Ability to work flexibly as part of a shift rota.
• Full UK driving licence and access to a vehicle.

Desirable:
• Good IT skills and confidence using care management software, Microsoft Office and digital record systems.
• Previous experience creating rotas, care plans and risk assessments.
• Experience supervising or leading a care team.
